Nearly 1 in 20 US adults over 50 have fake knees (AP)

CHICAGO ? Nearly 1 in 20 Americans older than 50 have artificial knees, or more than 4 million people, according to the first national estimate showing how common these replacement joints have become in an aging population.

Doctors know the number of knee replacement operations has surged in the past decade, especially in baby boomers. But until now, there was no good fix on the total number of people living with them.

The estimate is important because it shows that a big segment of the population might need future knee-related care, said Dr. Daniel Berry, president of the American Academy of Orthopedic Surgeons and chairman of orthopedic surgery at the Mayo Clinic in Rochester, Minn. He was not involved in the research.

People with knee replacements sometimes develop knee infections or scar tissue that require additional treatment. But also, even artificial knees wear out, so as the operations are increasingly done on younger people, many will live long enough to almost certainly need a second or even third knee replacement.

The new estimate comes in an analysis being presented Friday at the academy's annual meeting in San Francisco.

"These data are sobering because we didn't know what an army of people we've created over the last decade," said Elena Losina, lead author of the analysis and co-director of the Orthopedics and Arthritis Center for Outcomes Research at Harvard's Brigham and Women's Hospital. "The numbers will only increase, based on current trends."

Replacement joints can greatly improve quality of life for people with worn-out knees, but they're not risk-free and it's a major operation that people should not take lightly, she said.

Modern knee replacements in the United States date back to the 1970s. Since then, advances in materials and techniques, including imaging scans to create better-fitting joints, have made the implants more durable and lifelike, surgeons say.

Losina and colleagues came up with their estimate by analyzing national data on the number of knee replacements done from 1998-2009, U.S. census data, death statistics and national health surveys.

For example, in 2009, more than 600,000 knee replacement operations were done nationwide. The study estimate includes people who had knee replacement operations that year and in previous years who are still living.

Overall, 4.5 million Americans are living with artificial knees. That includes an estimated 500,000 who have had at least two replacement operations on the same knee.

Knee replacements are most common in people older than 80 ? 1 in 10 people in this age range have them, the study found. Though they're less prevalent in people younger than that, there are still more than half a million Americans in their 50s with the artificial joints, and based on current trends, operations in that age group are expected to increase.

According to the federal Agency for Healthcare Research and Quality, knee replacements tripled in people ages 45 to 64 between 1997 and 2009.

Doctors think two trends have contributed to that increase: the nation's obesity epidemic and amateur athletes who don't adjust workouts to spare aging or even injured joints. Both can lead to or worsen arthritis, the main reason for replacing knees.

Donna Brent, 63, is in the latter category. The Deerfield, Ill., administrative assistant says decades of racket ball, tennis, softball and other sports took a toll on her knees, but she got used to living with the pain, even when she became bowlegged and developed a limp. When pain "started getting in the way of some of my sports," she gave in to her doctor's advice and had the operation last June on her right knee. She said she feels better than ever, is back to exercising and plans to resume tennis and softball when the weather warms up.

During knee replacement operations, surgeons slice off a small portion of the worn-out surface on the ends of both leg bones that meet at the knee, then implant an artificial joint usually made of plastic or metal. Typical operations last about two hours, require a few days in the hospital, and cost roughly $40,000.

Artificial knees generally last 15 to 20 years. While some are promoted as lasting 30 years, these estimates are generally based on use among older people more sedentary than baby boomers who expect new knees to let them be as active as they were before surgery. Sometimes that's possible, though doctors often discourage knee replacement patients from engaging in high-impact sports including jogging.

Here Is What The New Health Insurance Labels Will Look Like - The ...

Back in August, we told you about how the Dept. of Health & Human Services was finalizing a template for new health insurance labels that would attempt to make it clear what a potential customer was buying and what sort of coverage they would receive.

Today HHS revealed the final format of the label. Above is what the first page of the 6-page template (see the whole PDF here). Subsequent pages cover costs and limitations for in- and out-of-network care for a variety of common expenses like doctor's visits, prescriptions, tests, mental health, substance abuse treatment, prenatal care, dental or eye coverage.

"All consumers, for the first time, will really be able to clearly comprehend the sometimes confusing language insurance plans often use in marketing," said HHS Secretary Kathleen Sebelius. "This will give them a new edge in deciding which plan will best suit their needs and those of their families or employees."

Insurers are expected to begin using these labels in their marketing and sales materials starting around September 23, 2012.

"A driving force behind the Affordable Care Act was to make the health insurance market work for consumers," said Lynn Quincy, senior policy analyst for Consumers union, the policy and advocacy division of Consumer Reports. "The new Summary of Benefits provides consumers with important insurance information in a standardized way for the first time. Our consumer testing showed that consumers dread purchasing insurance largely because they don't understand it and current health plan documents are insufficient. This rule is a big step in helping consumers better understand and evaluate their insurance options."

The labels all use a common glossary (PDF) so that insurers can not say "that's not what we meant by 'emergency services.'"

Quincy adds, "By making the terms of health insurance plans easier to understand, consumers are less likely to find themselves in health plans that don't meet their needs. We appreciate that HHS recognizes consumers would benefit from additional coverage examples."

For instance, if someone believes they will appear awkward and strange at a party (negative thought), he might mistakenly assume everyone will dislike him because of his awkwardness. A prediction of the worst-case scenario and jumping to an irrational conclusion are the distortions caused by the initial thought.

The basic outline is to distinguish negative thoughts, and to learn to challenge them and exchange them with positive ones. The last step can be very difficult, for the negativity might be reinforced by lifelong patterns. Indiana 'right to work' law: what it means for the pro-union Rust Belt

Indiana's new 'right to work' law is the first of its kind in the Midwest. But amid the region's disputed union issues, will the right-to-work law mean more jobs or lower wages for all workers?

After a protracted battle, Indiana in early February elected to make union dues optional for workers in union jobs. It's the first state in more than 10 years to adopt a so-called right-to-work law and the first state ever in the industrial Midwest to go that route.

What this means for Indiana is hotly debated. The law's backers expect it to attract more businesses and jobs to the state, and its detractors warn of lower wages for all workers.

The new law takes effect amid a climate of contention around union issues in other Midwestern states ? with mixed outcomes. In Wisconsin, a Republican takeover of state government in 2010 led to a new law restricting collective bargaining for most unionized public employees.

But in Ohio, voters last fall turned back a similar measure that had been approved by that state's GOP-led lawmakers. Governors of both states, along with Gov. Rick Snyder of Michigan, have said they don't plan on moving toward right-to-work legislation.

Despite disavowals from these governors, it remains to be seen if other states might follow Indiana's lead, says Martin Malin, director of the Institute for Law and the Workplace at Kent College of Law in Chicago. "It's almost impossible to read the political momentum" because, he says, "as union membership declines the political calculation changes."

Inside Indiana, both sides agree that the new right-to-work law will have a significant impact, though they disagree over whether the positives outweigh the negatives. The law specifically rules out labor contracts that require workers to pay a fee for union representation.

Unions in the state say the law may not hurt them right away, but they foresee a corrosive effect over time on union clout.

"It's going to be a while before we see the effects," says Ricky Wayman, an electromechanic at Verallia, a maker of glass beverage and food containers in Dunkirk, Ind. Mr. Wayman echoes the concerns of most union workers in Indiana who say that the measure's real purpose is to drive down union wages and, with them, the unions' political clout.

"Over the course of time we're going to have declining membership. New hires are going to see it as a way to keep a little more money in their pockets, which, in effect, will deplete local union funds," he says. It will deplete bargaining power and limit any kind of political action, he adds.

Proponents of the law see right to work as a pro-business effort to attract companies to the state, which will ideally lead to new jobs.

Many companies looking to relocate will rule out places that do not have right-to-work laws, says Rob DeRocker, an economic development consultant in Tarrytown, N.Y.

"The lack of a right-to-work law suggests that the mentality at the government level is that it's not a business-friendly state," Mr. DeRocker says. "Rightly or wrongly, it's an issue."

Right-to-work supporters also say the measure promotes the individual liberty of workers, giving them the choice whether or not to join a union ? and perhaps more important, pay membership dues.

Sprint loss widens on iPhone costs (Reuters)

(Reuters) ? Sprint Nextel (S.N) posted a bigger loss, reflecting the higher costs of selling Apple Inc's (AAPL.O). But the loss was smaller than expected because its signed up fewer subscribers than expected.

Since Sprint subsidizes the cost of some of its phone sales, its costs rise and profit dwindles the more customers it wins. But since subscriptions fell short of expectations, its loss was smaller than expected.

Sprint's loss was 35 cents per share excluding unusual items compared with Wall Street expectations for a loss of 37 cents per share, according to Thomson Reuters I/B/E/S.

Its profit margin based on operating earnings before interest, depreciation and amortization (OIBDA) fell to 9.5 percent from 16 percent a year earlier but beat expectations for 8.6 percent, according to eight analyst estimates Reuters compiled.

"It's still unbelievably depressed and subscribers were below expectations," said Roe Equity Research analyst Kevin Roe who also noted that Sprint's targets for the full year were not particularly impressive.

The margin decline was hurt by the hefty cost of selling the iPhone.

Sprint's rivals Verizon Wireless and AT&T Inc (T.N) also struggled in the fourth quarter with rising costs.

Sprint added 161,000 total net subscribers in the quarter compared with the average expectation for 272,000 additions from eight analyst estimates compiled by Reuters.

But it sold 1.8 million iPhones in the quarter, 40 percent of which to new customers.

Sprint's loss widened to $1.3 billion, or 43 cents per share, from $929 million, or 31 cents per share, a year earlier.

Revenue rose to $8.72 billion from $8.3 billion and was slightly ahead of Wall Street expectations for $8.69 billion, according to Thomson Reuters I/B/E/S.

Sprint forecast full-year net service revenue growth of 4 percent to 6 percent and forecast 2012 adjusted OIBDA between $3.7 billion and $3.9 billion.
